Thyroid Disease/Cancer and/or PCOS

I have been treating with an endo because I have the symptoms of PCOS (Polycystic ovarian syndrome). I am 28 but at about 18 I started to develop facial hair & it has gotten worse thru the years. I have never experienced any ovarian cysts and was able to conceive without trouble. Also, I have weight gain problems and difficulty losing weight. I am highly irritable, constantly tired, lazy, small appetite, wake up with a congested sinus daily, monthly neck pain such as a pulled muscle & constantly hot so I keep my AC at 68. Every month before my period I get something similar to a kiloid cyst along my groin that swells up so bad it hurts to close my leg & sometimes it needs to be drained because it is develops a bit of pus but it disappears right after my period. After my init visit 9 mths ago, my blood tests showed normal creatiane & testosterone hormones & normal thyroid levels except that my thyroid antibodies were high & my vitamin D was extremely low.  My doc felt that my thyroid was enlarged & I had symptoms of PCOS so he prescribed meds anyways. I was given spironolactone, metformin, synthroid & vitamin D 10,000mg.  After a mth, vitamin D, my vitamin D & antibody count was normal but my thyroid itself was still enlarged. Fast forward 9 mths of meds & my vitaminD is once again low despite milk & sunny days & although my hormones & TSH levels continue to be normal, my thyroid is even further enlarged.  Twice a month I get horrible neck pain that lasts about 1 week to go away, I cant step into a humid environment because I get congested and an immediate sinus headaches that turns into a migraine with dizzy, short of breath & nauseous. I have also been experiencing daily difficulty breathing as if my lungs are not getting air fast enough but not in the same manner as an asthma attack. I feel my pulse racing when I dont get enough air. The bp is normal (110/70) but my pulse rate is btwn 107-110.  Doc wants thyroid ultrasound. What could be the cause of my problems.

Would see an endocrinologist to sort this out but if the TSH is normal then the generalized symptoms would not likely be due to thyroid.  Ultrasound is appopriate if the thyroid is enlarged (but a large thyroid in the absence of abnormal TSH would not explain the other symptoms).

Also, I forgot to say that my sex drive has severely diminished in recent years as the hair growth gets worse
