When to have thyroid surgery

When should thyroid nodules be removed? Blood work was good, biopsy was benign with 5% room for error.  There is a total of 4 nodules, 1 is 2 cm, and another is 2 mm.

Primary indications for surgery: suspicious FNA or enlarging nodules with symptoms.
