goiter with elevated anti-tpo antibodies

I have a multinodular goiter with a large dominant nodule on the left side.

Here are my lab results:
T4,T3, TSH = Normal

anti-TPO = 280 (normal 0-60)
anti-TGB = 70 (normal 0-60)

I suspected to have Hasi's because of the elevated anti-tpo levels, but I talked to my endo and he told me that my nodules are very large for it to be hashi's and he did not prescribed any medications.

Is my tpo antibodies high enough to be hashi's? Will I be hypo eventually?

Looks like Hashi with future risk of becoming hypothyroid.
Not sure what is meant by "nodules very large to be Hashi"
If TSH >2-3, would usually start medications in this setting.

Thanks Dr. Lupo

The ultrasound says that I have a large well defined nodule measuring  3.5 x 2.5 x 2.25 cm solid, highly vascular, mildly echogenic in the left lobe.

There are also several ill-defined hypo- and hyper echoic nodules in left and right lobes.

I asked my endo to give me levothyroxine to reduce the size of the nodules but he insists that my thyroid levels are normal and I don't need it now. He also mentioned that hashi does not usually cause a nodule of this size.

Currently, my TSH = 1.34, FT3 = 3.45 and FT4 = 1.00 . Do you suggest taking levothyroxine? Is it likely to be cancer?
