no options

Due to some difficulties swallowing and feeling like I have a turtleneck  on I had a scan done.  The ENT said I had no options except removal, a biopsy would be pointless since it needed to come out.  Following are the scan results.  R lobe 53x24x26 mm L lobe 57x27x35 mm.  Multiple bilateral nodules, largest on R midgland 17x16x15 mm.  L lobe nodule 36x27x28 mm.  Lower pole nodule on L containing calcifications 12x11x12.  Additional lower pole R 13x10x9 mm. It has echogenicity and a few faint microcalcifications. Upper gland level on R 16x13x12 mm.  A little concerned no further tests are recommended but maybe he is correct and  a thyroidectomy is the best route. 2 months ago my TSH was 0.6. Surgery is scheduled Nov 4. Your opinion would be appreciated.

Sounds like you have compressive symptoms due to goiter.  With the TSH of 0.6, thyroid medication (or adjustment of) will not help.  We do consider pre-op FNA of conspicuous nodules as if there is a pre-op diagnosis of cancer, then the extent of surgery may change to include lymph node dissection.  Also, the entire neck should be evaluated with ultrasound to identify any abnormal lymph nodes.

Thank You, this all seems quite scary. I am finding it hard to believe that this is happening and I feel very out of control right now.  All this from a simple question about feeling choked. This is all new to me, I got the feeling he feels pretty sure it will be cancer.  I have never taken any thyroid meds. I will ask about ultrasound on the lymph nodes. Many thanks.
