please DR I need YOUR opinion


Please, dr lupo, I really need an opinion from you, I have been waiting for 3 weeks.i understand that you cant answer all questions. And I do respect that.  But I am desperate.
My dr wants to remove my thyroid, without taking other blood examples than TSH.which was 2, 0 (0.35-4.94) 2 years ago.and the nodule have been growing 3cm the last 2years.
I have this burning sensation in my lower neck, and painful ear and side of neck.


FNA . Cystic hemorrhagic and colloid nodule with follicular cells.      I am literally lost,,,,,, please any suggestions  would be appreciated. 

The Ultrasound Report. Uneventful FNA of heterogeneous nodule in left thyroid gland.

Here is my latest report. 

Right lobe, 11×11×31mm  left lobe 29×20×31mm,  Isthmus7mm The left lobe is seen extending down wards  till the level of the upper left clavicle. The lower half of the left lobe is seen replaced by a well defined nodule measuring 41×26mm 
Showing heterogeneous echo texture with multiple areas of breaking down causing cystic changes. Duplex study reveals diffuse increased vascularity being more evident at the lower half of the left lobe. Associated with ectatic left middle thyroid artery .Intact carotid sheath. No retrosternal extension.  the trachea is central, no enlarged cervical lymph nodes.

Anything else to be alarmed about?cystic changes,diffuse increased vascularity.?

The 4.1 cm nodule is fairly large and appears to be extending into the substernal area.  If there is not a significant fluid component to be drained and/or treated with percutaneous ethanol injection, typically for nodules like this that are growing and extending substernally as well as greater than 4 cm we do recommend surgery.  This typically would be a removal of the left lobe without removing the right lobe.  However I cannot get more specific advice without doing/seeing the ultrasound
