swollen lymph nodes and general pain

hello,

i'm 30/female/silent thyroiditis/112 mcg of synthroid.  

sono results from june: right lobe: 4.7 x 1.9 x 1.4, left lobe:  4.1 x 1.3 x 1.  numerous small lymph nodes on both sides of the neck near the thyroid.  largest node is just below the right lobe and measures 1.3 x 0.7 x 0.4.  doc's impression:  heterogeneous thyroid suggestive of thyroiditis.  multiple lymph nodes in the neck.  difficult to distinguish some of these from parathyroid lesions.

is the lobe size variation normal?  is the lymph node size abnormally large?

i had another sono done today and am waiting to get results.  if there are still swollen lymph nodes, what would be the next step?  and what would that mean?  is there a possibility of cancer/lymphoma?

i've been getting intermittent stabbing pain around my thyroid and upper chest.  i'm also getting intermittent ear ache-like pains.  could this be from the thyroiditis?  or the swollen lymph nodes?  i'm very worried.  this whole process is dragging on and on.  it's been 5 months of test after test, medicine increases, doc appointments, weight loss, weight gain, exhaustion, etc.  i just want this to resolve so that i can stop worrying and move on.  my next endo appointment is nov 14.  should i reschedule?

I had a total thyroidectomy back in January (2007) because of a large nodule.  A very small amount of well differentiated papillary cancer was found in the nodule.  But because it was so small at the time it was decided radio iodine treatment was unncessary.
I am not around sick people alot that I know of, I don't work in a school or hospital or anything.
Will a ct with contast show different things than a mri with contrast?  I had a MRI with contrast already that showed the location of all the enlarged lymph nodes.
You said yours are partially due to thyroiditis, do you still have your thyroid? Is it possible to have thyroiditis without having a thyroid?
Is your pain just in one side or all over?  My pain throughout this whole three month ordeal has consistently been in the left side of my neck right under the jaw line, at its worst point I also had muscle pain shooting down my left arm and in my shoulder, they said likely because the nodes were hitting nerves or I caused muscle spasms by holding my neck funny to try to compensate for the pain.

hello.

why did you have a total thyroidectomy?  how long ago?  what's your diagnosis?

are you around sick people a lot?  where are the swollen nodes?

i think my nodes are a combination of having sick kids and having thyroiditis.  my ent doc felt a couple of small nodes, but the mri and sonos that i had lead me to believe that the large ones are not in palpable locations.  therefore, i'm going to take the films to the ent so that he can determine the locations and severity (if any).

i would suggest asking your endo to order a ct with contrast to put your mind at ease.

what did you mean when your doctor said your swollen lymph nodes could have been caused by thyroid med?  Did he/she actually say this?  Did they say why?  Are you on synthroid?  I have been having extremely painful and enlarged lymph nodes in my neck and am on synthroid, and am wondering if that may be the cause?  I don't have Hashi, I don't think I can actually, because I have had a total thyroidectomy.  So I am thinking maybe the med theory is right...?

I had the RAI ablation in 2004

Yes - I had the MRI w/ contrast on my lymph nodes.

Stop by the community forum too - others there can also give alot of information as well as support.

hello.

what do you mean by "thyroiditis after ablation still confuses me"?  did you do the radioactive iodine to kill the thyroid?  or do you mean that occassionally your thyroid starts working again, then goes into overdrive and then thyroiditis?

thankfully, i didn't get any bad news from the mri.  no news is good news i guess.  however, just more of the same...lots of swollen nodes.  i went to an ENT doc who said that a CT or mri with contrast would've been more informative.  i basically wasted my time getting an mri without contrast.  i'm going back to the ent at the end of november with the films so that he can analyze them because the radiologist never identified the locations of the large lymph nodes in any of the reports.

i'm still trying to get regulated on the synthroid.   i'm on 112mcg, and i still don't think it's enough.  i go back to the endo in 2 weeks for the routine blood check.  thankfully, i feel soooo much better than when my tsh was 55.86.  unfortunately, my weight hasn't decreased.  when i was hyperT, i lost 5 lbs and was at my target weight.  when i went hypoT, i gained 10 lbs overnight.  i would at least like to lose the 5 lbs that i've gained overall.  i just feel like i'm aging already, and i'm only 30.  yuck.

Yes I have Hashi and thyroiditis after ablation still confuses me.

I swell on the right side of my nodes- right under my jaw line to my ear. Sometimes I notice it happening more when I am extremely tired or feeling run down. Also if I feel a stressful situation coming on too.

Hwo did the MRI go?

what kind of thyroiditis?  do you have hashis?  my endo said my thyroiditis was silent, but who knows.  i'm so frustrated and tired of this whole process.  it's been ongoing since april, and i feel like it's escalating instead of getting better.

this lymph node issue has really stressed me out.  i have two little girls, 4 and 2.  i can't bear the thought of cancer.  yesterday was a rough day because my endo ordered an mri.  i guess i'll get an answer, at least.

did you have a lot of swollen lymph nodes?  the only thing i can figure is that my girls are infecting me with the colds and such that they pick up from daycare.

I suffer bouts of thyroiditis and my lymph nodes will swell on occassion. I don't know why they swell and my doctors go up and down about it to like saying it a virus - or stress - thyroid med something like that.

I know the worry on the lymphoma thing as I felt that too. Finally for peace of mind my wonderful new doctor ordered an MRI of the nodes to rule out the fear I had on Lymphoma.

The scan came out negitive.

For peace of mind  - can you suggest to have your doctor run that MRI ??- trust me - it puts your mind at ease after it is done.

i keep trying to talk with my endo about this, and she hasn't returned my call.  therefore, i'm a nervous wreck.  what do you think about these results?  should i be worried about lymphoma?  or is this normal with thyroiditis?  when will the lymph nodes return to normal?

my new sono results:

the right and left lobes are normal in size.  there are no nodules and the echotexture pattern is more homogeneous than previously seen.

On today's exam a small nodule is demonstrated in the right side of the isthmus measuring .4x.2x.3cm.  This was also present on the prior exam and is not changed.

Several lymph nodes are again identified bilaterally having morphologic appearance of normal lymph nodes, however their size and multiplicity would raise the possibility of pathologic adenopathy.  the lymph nodes vary in size from approximately 1.5cm to 2.2 cm.  similar findings were also seen on the prior exam.
impression:  
there is a subcentimeter nodule in the right side of the isthmus which is stable.  
bilateral lymph nodes are present with normal appearing morphology, however their size and number raise the possibility of pathologic adenopathy.  
if felt clinically needed the finding could be further assessed with a contrast-enhanced ct of the neck.

The ultrasound is typical of thyroiditis -- which commonly has small lymph nodes adjacent to the thyroid (which can appear like parathyroids, but they are lymph nodes).

Cancer is not likely, but cannot be completely exlcuded.

Thyroiditis may cause ear pain -- but "silent" thyroiditis is usually painless.  However any thyroiditis could potentially cause some pain.  Sometimes the thyroiditis will recur quickly (within 2-3 months) and drag out the process for several months.

The high lymph count would most likley be virus -- ask you general doctor about this and if it needs repeat.

also, my last cbc showed high lymphs at 49.  is this significant?  should i get that redone?
