14 year old male with low free t4 and swollen glands

My 14 year old son has had two blood tests over the last couple of months show low free t4 and normal TSH. During this 2-3 month period he has also been lethargic and had swollen glands on the right side of his neck. He is now saying that it feels uncomfortable when he turns his head to the left. Until recently he was very energenic and healthy. We are trying to get an appointment with a pediatric endocrinologist and I was wondering just what tests I should be expecting them to want to do. Are there any additional blood tests that he should have done? Should I request an ultrasound if they don't recomend one? Any other tests?

Would go along with whatever the pediatric endo suggests -- they will likely do ultrasound and more comprehensive thyroid tests including possible TPO antibody.
