Hashi with large goiter(s)

Hi I am a 37 yr old mother. Until about 4 years ago I was fit, healthy. I started gaining weight & have now gained approx 50 pounds cannot seem to lose it or stop gaining.  I have felt tired fatigued and just plain exhausted.
Should mention, reason I went to dr was due to my heart racing.  This occures 1 or 2 times per year, I will actually feel a buzz in my chest my head will spin then my heart races for about 15 min.  This started happening about 1 X per month, was scaring me so I went to the dr. after wearing a monitor for 30 days they captured my heart racing from 200 - 250 this is while I was sitting (inactive) Presently taking Toperal for this seems to be working. Anyways dr noticed a large bump on my neck had blood work ultrasound biopsy scan. Was told I have hashi/hypo large goiter. Now taking synthroid .1mg for approx 6 mths Have always had bad menstral cycles extremely heavy body aches all over, extreme headaches. Went to dr this last time 2 weeks ago told him I could not take this anymore, and how tight my neck felt. Also I have a bump on my chest below my neck I did not mention at my last dr visit, could hardly keep my head up felt so bad forgot to mention. I did tell him I feel tight hard to turn my head becoming out of breath easy feel bruised hard to swallow, blood test says TSH is normal another ultrasound says goiter is bigger need biopsy again & specialist. He is sending me to an ENT. Should I worry about the bump on my chest I tried to tell the nurse she just said he is sending you to an ent. we'll call with appt Can all of this really make u feel so ba

Sounds like it is time to consider surgery to remove the thyroid - your symptoms in the neck are called "compressive symptoms" due to size of the thyroid.  The ENT will talk to you about surgery.  Sometimes a repeat biopsy is helpful in determining the extent of surgery (ie, more surgery if there is a concern for cancer).  Tell the ENT about the bump on the chest - it may not be related to the thyroid.  The synthroid sometimes helps shrink the thyroid but does not always work - as you are finding out....if you have hashi, then synthroid is helpful to keep the TSH around 1.

Thank you for your response. I have an appt this Fri with an ENT.  I just really concerned because I feel so badly, I am always exhausted sometimes close to tears and I have never felt like this before.  I have never sufferd from depression or anything.  Everything that I do seems to exhaust me.  I work full time have 4 kids who are involved in alot of activity keeping things going is becomming increasingly difficult.  The bump as I called it is more of a hump and covers approx. 1 1/2 inches it only sticks out maybe 2/8" but you can see it.  I just have such intense pressure in my neck and chest.  Are there any other questions that I should be asking my dr? I do not know all of my lab results I do know that my TSH was I think 14 before I started Synthroyd now it is 1.  I know that the first ultrasound & Scan showed the goiter to be 2.5 cm.  Now they said it is bigger also, (you know how they will not tell you anything when they do the ultrasounds.) they were saying things like "There is another one, and another we need to measure this one and that one etc."  Just enough to make me worry more.  Any advise would be so greatly appreciated.  Thank you.
