High TSH and T4

Diagnosed hypo in 1991 - on 150mcg thyroxine since until 5 years ago when dose reduced to 125mcg.  2003 had ASD repair and single bypass and on 75 mg aspirin; 40mg lipitor and 25mg losartan.Labs OKish until last lab test:

TSH 7 (5 upper lab range)
Free T4 25 (upper lab range 23)

Also cholesterol jumped from 4.7 six months ago to 6 now.  I do not understand why TSH high with high T4 and could this have affected the cholesterol.  Also a few weeks ago had one sided throat pain with neck on that side painful to the touch - might that be thyroid related?  Thank you for your time.

Would repeat the labs to confirm,  the TSH may be more accurate than the free T4 so the slightly high T4 is not that much a concern.  Mild hypothyroid (TSH 7) may increase cholesterol, but with the history of coronary disease the cholesterol needs to be tightly controlled.  So, if repeat TSH still high, then consider increase to 137mcg and recheck thyroid and cholesterol in 6-8 weeks.  May need more cholesterol med as well.

Rarely would a high TSH and high Free T4 together represent a separate problem -- if this pattern continues, would also check free T3 and if all elevated, need endocrine evaluation.
