Hurthle changes are common in thyroid FNA samples and are associated with Hashimoto's, Graves and less commonly toxic nodules.  The key is the pathologist reading the slides -- should be an expert (to avoid unnecessary surgery due to over-concern about the presence of hurthle cells).  Usually lesions that are "suspicous for hurthle neoplasm" have >75% hurthle cells and no lymphocytes and little colloid -- these carry a 20% risk of cancer.
