I am on 175mcg. of Synthroid & my T4 is still low (from some "harmless" low antibodies or low something),FT4 is getting LOWER with increased dose of meds, NOT higher! Also, but went 1 yr. being "postmenopausal", had a "period" in Jan. Went to the doctor, did tests, FSH showed that FSH level WENT DOWN FROM 56.5 TO - - 8.5 which "restarted" my periods. Now look at all my results from Jan. to now:
**See changes!
JANUARY 2,2007
TSH: 0.45 (0.34-5.60) (good)
T4: 4.8(l) (6.1-12.2) (too low)
**FT4: 1.08 (0.58-1.64)(good)
**FSH: 8.5 (normal menstruating female)(A++)
FEBRUARY 28, 2007
TSH: 0.41 (0.34-5.60)(good)
T4: 6.0 (6.1-12.2)(better but still low)
**FT4: 0.89 (0.58-1.64)(LOWER WITH INCREASE! NO ENERGY-return of symptoms)
** FSH: 55.1 (Postmenopausal Females 20-138)(HORRID)
Why is my FT4 LOWER this time?? Why is my FSH so high again. I see Endo on 12th. Is it adrenal or pituitary?? I'm VERY CONSTANT about the way I take  meds, I keep them in cool, dark place.  I even wait 2 hours at least before I drink coffee, juice, anything....Doubtful it's interference of other meds
Had a bad flu the week I took the test.  Any correlation?  Thank you!  I am waiting for results to return on the FT3.  The lab didn't send them back yet, but will post next week

This is not likely adrenal or pituitary.  The FT4 is more accurate and can vary a little depending on the timing of the synthroid dose.  The TSH looks fine so this is likely stale and possibly symptoms are more related to being peri/post-menopausal.
