Thyroid Nodules W/Hashi's

Long story short; palpable nodule discovered on well woman exam 2006, 1st endo referred to; dx; Hashi's my TSH was 6.4, Thyroid peroxidase ab were 331 normal was <35. 10/06 u/s of thyroid; heterogenous appearance, rt isoechoic nodule 2.68 x1.45 x2.14cm, lft lobe unremarkable, fna was a poor sample but noted; possible follicular lesion.  I went to another endo for 2nd opinion; tried thryoxine suppression, my tsh went down to 0.11 on Synthroid. What bothers me is the following; repeat u/s done this month showed; decrease in size of prior nodule; 2.33 x 1.81 x 1.33, but now it notes I have a lft sided nodule 1.88 x.76 x..64.  Could this have been missed previously? or can  nodules come up even when on Synthroid? I am scheduled for another bx this month.  What treatment plan do you see?  Note; family hx; aunt had goiter, sister has goiter, no thyroid cancer. I am 42 yrs old.

Nodules can come up even on synthroid. Hashi is sometimes difficult with regard to ultrasound -- there were likely two nodules from the beginning -- but the inflammation can make it difficult to see the borders of nodules and sometimes there are 'pseudonodules' due to the irregularity seen on ultrasound.

The R nodule is decreasing (presumably) in size, but now there's a nodule on the left -- the dimensions are suspicious for a pseudonodule (othewise it's like a short string-bean in terms of dimensions)

There is not one right answer here, I would repeat US in 3 months and if there are clearly bilateral nodules, FNA both of them to get good samples and to have peace of mind this is all just hashi.
