acth deficiency

My 80 year old mother-in-law has suffered from Cushings Disease for years. Recent tests show no acth production. Her endocrinologist wants her to stay on 15mg of cortisol per day (10/5). She has additional health problems--bulging disk, arthritis, recent knee surgery. She has terrible fatigue and weakness. Is this dosage sufficient? Should she be on additional replacements? She received the lab report in the mail with no follow-up from her doctor and no idea what is next. They haven't checked her pituitary or other areas.

20mg/day is the usually minimum dose for a patient whose body makes no cortisol -- but, cushings disease is an over-production, so perhaps she's been treated for this and now has an under-production?  The symptoms are not-specific, but can be seen in adrenal insufficiency.
