I cannot make specific recs and if you are having chest pain and air hunger it is likely from the profound hypothyroidism, but would suggest having a heart evaluation as soon as possible to be certain.  This degree of hypothyroidism can cause blood pressure elevation and make the heart slow/irregular.  The levoxyl therapy is important and clearly needed but will take some time to take effect.  The dose if you are <60 years old without previous heart disease is usually about 0.7mcg/pound but can vary from person to person -- with your previous experience at 125mcg, a dose of 100mcg may be reasonable but only your doctor can decide this.
