ectopic source of cortisol?

I have written a few times about the endocrine testing I have undergone.  I'm trying to understand why my doctor is not testing further.  If a serum cortisol and 24 hour urine are both elevated but an overnight dexamethasone suppression test is fine would this indicate an eptopic source of the cortisol production?  Brain/Abd MRI essentially normal CT scan of abdomen and pelvis normal. Urine 5-HIAA ok.  Shouldn't the cortisol elevations be followed up on.  

If not, what is the long term effect of cortisol elevation on the body?

No - ectopic producers are usually not suppressed with decadron.
