The question is regarding my wife, her thyroid gland removed before 8 years , and she is taking the medicine eltroxing before 100mg , and later incresed to 200mg.our daughter is already 2 years , now in last december she is pregnent again and we check the tst level and t3 and t4 , its all normal but t4 free some thing its in the edge of the recommended level, so the doctor told to reduce the daily dosage to 150 from 200mg. now in last week we check again the tsh (prgnencey almost getting 6 months)then the tsh is 60,now the second doctor is blaming us that why reduce the dosage and he is asking to increase again to 200mg. we just check in the last week just after 1 day to confirm the resul is right then the tsh is 36,now my wife is taking 200mg, and plan to check after 15 days. but idont understand why the tsh level its so high when the normall level is .5 to 5 . and the second thing only reduce 50 mg making so muche difference? is there any problem for my inborn baby ? what shall i do now to control the thyroid problem.

The goal TSH w/ pregnancy is 0.3-2.0 (in general).  Thyroid requirements increase during pregnancy - which explains why the decrease from 200 to 150 resulted in a dramatic increase in TSH.  Most pregnancies are not affected by this, but there is an increased risk of neurological development (low IQ), miscarriage, early delivery and other complications with hypothyroidism during pregnancy.  Testing closely throughout the rest of the pregnancy is critical to ensure things are stable on the 200mcg dose.
