fluctuating TSH, pituitary antibodies

My Free T-4 and Free T-3 continually remain almost exactly the same reading, over the years.
However, my TSH has fluctuated from 0.05 to about 11.7.  All this while remaining on the same
dosages of Synthroid and Cytomel.
My CK remains constantly elevated, and is currently idiopathic hyperCKemia.
I've read where anti-pituitary anti-bodies could be tested, and HGH an issue, (re-Rida Arem, MD).
Any thoughts or advice?  I see a new Endo soon and would like to be on top of the whole situation
if possible.

Anti-pituitary antibodies are being looked at on a research basis but I have not seen a commercial lab doing this yet.  There was a recent article suggesting an increased risk of pituitary auto-immune problems with Hashimoto's.  

Growth hormone testing is fairly straight forward - IGF-1 can screen, if low then Argining/GHRH stimulation can be done.

The fluctuating TSH may be due to other meds, GI absorption,etc - that is quite a range.  The high CK -- rheumatology evaluation has probably been done -- have you had  muscle biopsy ?  Thyroid problems can do this but if you have been on replacment for years and within that TSH range, it is not the likely culprit.

14 yrs. replacement and within those ranges constantly.
The other workups provide little clues, fluctuating elevated
Sed Rate, a bit of anemia.  Thank you.
