It's entirely possible to be sensitive to the fermentation (and even high sodium levels) in the soy sauce, the combination could be causing your irritation. Do you get a similar reaction if eating lots of vinegary things, ie pickels, vinaigrette salad dressings, or other fermented items (like mushrooms, cheese?) Could be you are sensitive to the fermentation/mold.
It's definitely MSG and be careful because even if the restaurant cooks MDG-Free, the bottled or packaged soy sauce, does not come from them and could very well contain msg.

I have the same problem with soy sauce. Once I read about the fermented soy sauce is not harmful, but now I strongly suspect that.
With all due respect: I'd do the testing before the asking.
I once waitressed in an Asian restaurant and one time asked the cooked whether anything had msg in it. This was because a customer who told me she has to go the er if affected was asking. The cook said, "Tell her, no." Then he and everyone else in the back rolled their eyes. Later they told me it can be in a lot of small ingredients but making an issue of it is just a silly American thing. I didn't know what to say to the customer so I gave her a sort of 'when in doubt, leave it out' kind of recommendation.
It was weird. You've got to take care of yourself, I guess.
I do have to say, though, it did cross my mind to wonder what someone who's had to go to the er over this issue was doing there in the first place.

You should ask to have them not use it when ordering.
Are you allergic to MSG or soy? You may find that is the problem..(Careful! It can worsen after multiple exposures...) One simple way to find out is to take a dose of benedryl the next time you develop a rash after eating it...If your "illness" is from a histamine release (allergic reaction), the benedryl will work within an hour... ~MM
