Higer TSH is it possible to go lower by self

Hy,
I had my last TSH checked at result is
TSH 5,340 ( 0,465-4,68)
My doctor checked it because os ovarian cyst and inregular menstrual cycle...
I had checked TSH level 5 _7 years ago
TSH     4,515 ( 0,470 _ 4,640)
Free T4  13,03 (9,14_23,81)

6 month later

T3       1,63 (0,92 - 2,33)
T4      74,54 (60,00 - 120,00)
TSH      2,83 (0,25 - 5,00)

And one year later

TSH   1.59   (0,55- 4,78)
T4    109.1  (58.1 - 140,6)
T3    1,6    (0.9 - 2.8 )
Anti tg  43  (<60)
Anti tpo 40  (<60)

So honestly im confused, i never take medication for Tsh.
I do take mirtazapine and xanax, last few years do to depression and anxiety..
Also i have high cortisol in the morning, that doesnt drop in the evening.
I did dex test , and it drops cortisol level.
And i did ct scan of adrenal glands which is fine..


Only i can say, i take medicine for depression and anxiety, an it doesnt work, i try few ad.
But im to sensitive for anxiety, like my body cant deal with little amount of stress..

I can find my self in most of syptoms of high tsh , but also for low tsh...

Is it normal to have my test result like a have, sometimes high sometimes low.

Next week im going to endo , and do ultrasound,
Can you give me advice, what test i should ask to be done.
I think it will be free T3 T4 and maybe they would check my cortisol...
Im little worried bc my doctor told me to go for endo, to start drinking pills to lower Tsh,

I had never been diagnosed for any adrenal disorder

I had check in 2012
Cortisol am 1047 (138-689)

Androstendion 29,1 (1,7 -16,4)

Testosteron 5,3 ( 0,4- 2,74)

17 oh prog 5,5 (0,33-3,27)

Other hormons where in range
I dont know what happened to my body at that time , but most hormones i checked was elevated.
I didnt know about this forum then
I wish i had

17 oh prog that shows 10 , its after atch stimulation test.

They checked that for late on set cohgnital something, and told it was fine

You told me that cortisol and 17 oh prog dont match to be both high, what i had.
What that mean if they are elevated together..

So it will be good to check 17 oh prog

Can you give ne advice, my tongue is going bigger and bigger, i can see teeth mark on tongue,
What can i do to make it smallee before i go to doctor and do more test.
Its a little scary , i want to put my tongue out like dogs do :-///

Only i have TSH from last week

If you have time, i would like to understand more about cortisol i had before tested.

I was,even years ago feel week,and lots of others syptoms, and i allways thougt it was my mental problems, i take medication for deppresion and anxiety, and keep pushing my self to study, work and live.
When i know other facts that can elevated my depression and anxiety, i feel big help in understanding its not just my mind.

Your TSH from last week is higher than the upper reference range and indicates that you have hypothyroidism (under active thyroid).  This, typically, requires replacement thyroid hormones to bring your levels up to where they should be; however, you really need to have Free T4 and Free T3 tested, as well, because those are the actual thyroid hormones.

Cortisol is a steroid hormone produced by the adrenals. It serves a lot of functions in our body, including helping to control blood sugar levels, regulate metabolism, helping to reduce inflammation and assist with memory formulation.  It helps control salt and water balance and regulate blood pressure.

Ideally, it's higher in the morning when we're preparing for the day, but it drops as the day goes on and is lower at night, so we can prepare for sleep.

The first cortisol you list in 2012 was definitely, too high, but you don't know what it did later in the day, because you only had the one morning cortisol.  

The test taken with the dex was to see if you have Cushings, which is an adrenal condition.  From what I can tell, the test was negative, because your cortisol level came down after you took the 1 mg dex.

Are you saying your 17 OH progesterone was 10.4?  Upper range varies by lab, but healthline.com puts the range at 6.06 nmol/L for adults...

Your tests all seem to have come out normal, except the cortisol and 17 OH progesterone... Cortisol is supposed to lower after taking the dex and it did each time.  

You had an ACTH test following the high 17 OH progesterone, but from what I can see, those results were normal; what did your doctor say about them?  17 OH progesterone is used in the production of cortisol and some sex hormones.  Typically, when 17 OH progesterone is high, cortisol is low; your levels of both, are high.  

Were you ever diagnosed with or treated for an adrenal condition?

Even though I've tried to explain the best I can, it doesn't change the fact that all of your tests are several years old and are pretty irrelevant now.  Considering that your cortisol levels were so far off, back then, it's even more important that you get current tests to see how things might have changed for the better or worse, now.  

Sometimes if we're under a lot of stress our body will produce a lot of cortisol for periods of time, but once the stressful period is over, we can recover on our own.  it doesn't always work that way, but "sometimes"...  We aren't usually so lucky with hypothyroidism and as I noted previously, when we become hypo, the adrenals kick in to try to take up the slack for the thyroid, so even if you might have made some recovery from a few years ago, your thyroid may put your body back under stress again.

If you answer for tsh 4,515 and free t4 13,03 , later i had check few times TSH and it was in range...



My cortisol i did in 2012
Morning 1047,00  (138-689)

Month later
Cortisol 7_9 am 433,1 (119 - 618)
         3-5 pm 446,4 (85 - 460)
No drop in evening

In 02.12 4 months later
Cortisol am   379.6 (171-536)
Cortisol pm   399.5 ( 64-327)

Acth am.  3.42 (1.6- 13.39)
Acth pm.  10.12. No referent renge

I take 1 mg dex in the evening,
And take cortisol test next morning

Cortisol 24.19 ( 131 _536)
Not sure for interval i think it wss for regular cortisol


Two momths later i had acth test , for cortisol and 17 oh prog ( bc i had high level of 17 oh prog)

Before acth injection

Cortisol am 801  ( 138- 714)

17 oh prog  5,6 ( diff inerval due to menstrual cycle, but i cant remmember )

60 min after acth injection

Cortisol 1507,00 no interval
17 oh prog 10,4 no interval



And last check of my cortisol was
October 2014
20 min i was relaxing with braunila in my vein
Cortisol am 587,9 (101,2 - 535,7)

And i colect urin 24 h
Cortisol (u) 216,0 (11,8 -485,6)

U24 diureza 1500 no ref interval


So that night i take small dexa test

Cortisol am  193,00 (101,2- 535,7)
Not sure if interval is for regular cortisol


Month later again smal dexa test

Cortisol am 25,4 (101,2-535,7)

You're not giving too much information, but it you're going by tests done in 2012 and 2014, they're no longer valid... A lost can happen/change in 3 years or 5 years.  We need to know what's going on now, so you'll need to get new tests done.  

Are the TSH and Free T4 tests current or are they from a few years ago, also? If so, those will need to be done again, as well.

I just realized that I didn't really answer your original question... TSH can actually fluctuate considerably, up to 70%, within a day's time... It's very important that when you have TSH tested, you also have Free T4 and Free T3 tested, as well.

Your TSH is higher than the upper reference range and your Free T4 is very low in its range at only 27%.  This indicates that your thyroid is not producing the hormones your body needs to function adequately.  

Anxiety is a common symptom of hypothyroidism (under active thyroid), which is what you have and could explain why your medication for depression and anxiety doesn't work.  Irregular menstrual cycles can also be a symptom of hypothyroidism.  Many symptoms can apply to both hyper and hypothyroidism, such as fatigue, anxiety, depression, heart palpitations, etc, so it's not really unusual to have, what you would consider symptoms of both.

Even though you've had the antibody tests (TPOab and TgAb) once, it would be a good idea to have them again, since it's possible that they simply hadn't ramped up yet when you had them before.  

What were your actual cortisol levels, with reference ranges?  When the thyroid isn't working properly, the adrenals kick in to help take up the slack.  This would not show up on a CT scan, but could show up in blood work, in the form of higher adrenal hormone levels.
