Will the doctor test you for cortisol and ferritin? If not, I think you should show her the list of symptoms that are related to being hypocortisol, and push for the tests. If that doesn't work, for cortisol are you willing to spend about $150 to get a kit for a diurnal saliva test panel of 4 tests from an online lab? Low ferritin can also cause the following symptoms.

The ACTH only assesses the functioning of the adrenal glands stress response. If there is an inadequate signal from the pituitary, the test would not show it. That is why you also need the cortisol test. Also, your low TSH could indicate low output from the pituitary gland. If the doctor cannot find the cortisol test result, then you need to do another. While waiting for your doctor to do something, if you start feeling really bad, I would just go to the ER and make sure they understand your symptoms and the possibility of hypocortisolism.

The best test for cortisol is the diurnal saliva cortisol panel of 4 tests at different times of day. Those check free cortisol. The test doctors usually will order is morning serum cortisol, which is total cortisol and not nearly as revealing, but better than nothing.

Although there are a number of other variables that can cause hypo symptoms, your FT4 and FT3 levels should not be the cause for your severe symptoms. Total t4 is now 11. 12 is the limit that's set by lab to be high its went up was 10 7

Can't believe the doctor would not know the TSI test for Graves' Disease. Also why would she think that this Free T4 should be up and Free T3 down? I think you are going to need a good thyroid doctor soon. With the symptoms you have and the limited test results it sounds like hyperthyroidism. So, I think the first thing you need to do is to get tested for the possibility of Graves' Disease. The definitive test for that is Thyroxine Stimulating Immunoglobulin (TSI). In addition, you need to be tested for the biologically active thyroid hormones, Free T4 and Free T3 (not the same as Total T4 and Total T3). For the low Vitamin D you need to supplement with probably 3000 IU of Vitamin D3 daily to get your level to at least 50 ng/mL. You also need to test for cortisol and ferritin.
