Also, I have a large isthmus, caused by Hashimoto's. Sometimes, it is hard to swallow pills, but eventually, it will shrink up when the antibodies are finished with it. I take 200 mcg Selenium daily and it helps with the swelling.

You have thyroid nodules and a large isthmus, common in patients with thyroid disease. They are recommending a fine needle biopsy. This hurt less than a trip to the dentist for me. They numb your neck and take out samples with a needle. It hardly hurts, but it does leave bruises for about a week.
None of your nodules are over 2.0 cm, so I wouldn't worry.
Have you been tested for Hashimoto's? I suggest getting the TGab and TPOab blood tests to see if Hashi is behind this.
Next, get the TSH, FT3 and FT4 levels tested to determine your thyroid levels.
Post those levels here when you get them if you want our opinions.
