Meds not working?

I found out only a year and a half ago that I have Hashimoto's Thyroiditis when my neck was swelling slightly and it was getting hard to swallow. We went to the endocrinologist and she said that I have this disease. Well now I take .75 milligrams of synthroid everyday and when it's 'that time of the month' again, my thyroid swells more than normal. It is troublesome because I take my medication everyday but my gland keeps swelling. It has me slightly worried because I cannot breathe comfortably or swallow easily without it hurting horribly. Is this because my medication is not working like it should be? I don't understand.

Have you ever had an ultrasound of the thyroid?
Do you have bloodwork done periodically to check your hormone levels?
