TSH w/reflex to FT4 is an instruction to the lab to analyze TSH and if it's out of range, they should then analyze Free T4. This is really an outdated way of testing, which shows that your doctor is not very up to date, but apparently, your TSH was 12.11, so the lab should have done a Free T4 test, as well. Do you have a result and reference range for that?
You need to talk to your doctor, because a TSH of 12.11 is over the reference range. It's an indication that you have hypothyroidism and either need to be started on replacement thyroid hormones or need your dosage increased, if you're already on thyroid medication.
When you talk to your doctor, tell her/him that you also need to have Free T3 tested, because Free T3 is the hormone that's used by every cell in the body and is just as important, if not more so, than Free T4.
