Surgery required

I have a single Dominant thyroid nodule in the left lower pole , with hetrogeneous echo strucutre and a solid one Ultra sound shows it's measurment in last October 20th 2008 was 4.1cm * 3.8 cm * 3.0 cm .The FNAC shows Follicular lession with a cystic change. Left Lobe is enlarged measuring 5.1cm* 3.6 cm* 2.5 cm . In 19th Feburary 2009 Ultra sound measurment of the tyroid nodule is 4.2 cm* 3.6cm * 3.8 cm with a enlarged left lobe . I just want to know do I need to surgery sooner or later due to enlargement of the left lobe
