Synthroid dosage

I have been taking Synthroid for about 9 years now and have my TSH tested at least once a year.  I just had it done this month and the result was 4.704, which is slightly above normal limits according to the nurse.  She told me my physician was going to keep my dosage the same, which is 100mcg, even though my physician noted at my exam that my thyroid gland was enlarged and I am having classic hypothyroidism symptoms.  Tired, hair is really thinning and very dry and brittle, dry skin, cold all the time, forgetfulness, and weight gain even though I exercise and watch my diet every day.  I read online that when a patient is on Synthroid the TSH goal is to be between 0.3-3.0.  Is this the target that most physicians try to keep their patients at?  I am confused as I am having problems but she does not want to increase my Synthroid.  Any advice would be appreciated.

A TSH of 4.7 is way over the new limits of .3-3.0.  Your doctor should be aware of the new limits and should be prescribing medication to alleviate your hypo t symptoms.  You would feel much better if your TSH was closer to 1.0.  And it would be even better if the doctor would also check your freeT3 and Free T4.   pep88 is right on with recommendation that you should find a new doctor that understands thyroid problems.

And the bottom line is that you don't feel well.  Feeling unwell is as important a symptom as the others you listed and should be important to your doc too!  And reason enough to seek another opinion!

I would totally seek a second opinion.  You mention that your thyroid is "enlarged"?  Why is your doc not concerned with this?  Is this an GP, an ENT or an Endocrinologist?  I would be making some changes in my healthcare provider if I were you...
