TSH & fT4 levels

We are adopting an infant (now 3-1/2 months) from Vietnam and received the second test results for TSH & fT4 this evening.  The first test results showed possible secondary hypothyroidism.

First results:  fT4 was 7.51 pmol/L (7.4-14.4) and the TSH was 0.05uUl/mL (0.5-7.6)

Second results (one month later):  fT4 was 13.2 (3.8 - 77.2 pmol/L) and the TSH was 4.3 (normal range 0.4 - 4.0 m/UL)

The T3 is now 3.2 (range 1.2 - 2.7 nmol/L)

Our Pediatrician (who is the only one handling Int'l Adoptions in the office) is out until next week.

Secondary to what?

I see subclinical hypothyroidism on your lab references which are not AACE. .3-3.0 is correct ranges so primary hypothyrodism seems to be logical.

At one time the test had shown (0.05) hyperthyroid. Was a TPO antibody done ? I ask b/c one month later hypo blood tests are shown.

Antibodies killing off thyroid tissue?

That then would give the secondary Dx you are saying.

I do know - other members here will had on - watch this post.

Thank you for your quick response.

The report shows Secondary Hypo T....Low FT4 and Low to Normal TSH.

I am not sure what a TPO antibody is.....do not see anything close to this on the report.

The doctor suggested we ask for a cortisol test but, unfortunately, we are told that they are unable to do this test.
