TSH Level on Near to Upperside

One of My Friend is having following Thyroid Profile

T3 127.6   (60-181)

T4 6.39     (4.5-12.6)

TSH 5.38   (0.27-5.5)

Should he take required to take any care or medicine. He is having some chest heaviness as symptom rest is fine & aged around 43.

Any Advise, should medicine is required in this case

It does look like your friend's thyroid function is beginning to decline and he would benefit from a dosage of replacement thyroid hormones.

It appears that the testing may be outdated...Do you know if the T3 and T4 are Free T3 and Free T4 or if they are Total T3 and Total T4?  It's best to test Free T3 and Free T4 as opposed to the Totals, which are considered obsolete.  It should specify on the lab report if they are Free...

It's, generally recommended that Free T4 be about mid range... your friend's T4 is only at 23% of its range, so that's quite a bit shy of what's recommended.  The T3 is still pretty good at 56% (recommended is upper half to upper third of range), but T3 often lags behind T4.

It's usually best to start with smaller dosages of medication and work up as needed.  Your friend will need to work with his doctor.
