Sorry if this is a dumb question, but, what is APD? Nexium is used for acid reflux/GERD.

What are your actual thyroid hormone levels?  Your symptoms could also be those of being over medicated with the carbimazole.  Fatigue, constipation and GERD are all symptoms of hypothyroidism.

If you have current thyroid hormone levels, please post them for members to better assess your situation.  Please be sure to include reference ranges, since these vary lab to lab, so must come from your own report.

Do you know if you have Graves Disease?  Or Hashimoto's Thyroiditis?  While Hashimoto's is most often associated with being hypothyroid, it often presents periods of hyper, as well.

Esomeprazole (nexium) and all other proton pump inhibitor have a black box warning - hypomagnesemia (magnesium deficiency). Symptoms of magnesium deficiency are many but include constipation, palpitations and fatigue. Not all hot nodules produce symptoms but your nodule may be the cause of palpitations and fatigue.  If a hot nodule causes hyperthyroidism, it can be treated with anti thyroid drugs, radioactive iodine, or surgery.

Taking acid blockers are detrimental to mental and physical health over time. Acid blockers were introduced to the market for short term use - 6 weeks at the most.  Causes of acid peptic disease (APD): Helicobacter Pylori bacteria (major cause - treatment: antibiotics), consistent use of pain relievers, prescription medicines known as bisphosphonates.
