Could you have the T3 and T4 levels switched? Are those Free T3 and Free T4 or are they Total T3 and Total T4? They aren't the same tests and don't give the same the same information.
We also need the reference ranges for the T3 and T4. Ranges vary lab to lab and have to come from your own report.
TPOab of 395 would be the basis for a diagnosis of Hashimoto's Thyroiditis and would be consistent with Dr Lupo's comments above.

Since I don't know what labs you've had or haven't had, I can't recommend a second opinion, just yet.
Have you had any blood work labs to determine thyroid function or is the ultrasound the only thing you've had done?
If you've had blood work, check your reports for things like TSH, Free T3, Free T4, Total T3, Total T4, T3 uptake, Thyroid Peroxidase Antibodies (TPOab), Thyroglobulin Antibodies (TGab), etc.

i don't know what look for on labs! docs Secretary said it was stable and there wasn't a mention of parathyroid or ectopic thyroid tissue which i have the report should i get a second opinion

I'm not sure it will do any good to continue posting your ultrasound results, here, because it's unlikely Dr Lupo will respond on this thread again. You might want to post on the expert forum.
Dr Lupo indicated Hashimoto's; have you had any antibody testing done that might confirm this?
Hashimoto's leads to hypothyroidism... do you have symptoms of that? Symptoms could include weight gain/inability to lose, constipation, fatigue, muscle/joint aches/pains, dry skin/hair, hair loss, as well as others.
Have you had thyroid hormone levels tested? Those would be Free T3 and Free T4. While TSH is not a thyroid hormone, you should get that tested, as well.
Do you have any indication of parathyroid issues? You might want to get calcium levels tested, as Dr Lupo suggested. Also get PTH levels tested, if you haven't already.

Generalized thyroid hyperemia which can be associated with thyroiditis. No discrete thyroid lesions identified.
Two small hypo echoic solid nodules inferior to the lower pole of the left thyroid lobe with differentials including a small cervical lymph nodes. One of these could also reflect a small parathyroid gland.

This sounds like hashimotos with either possible nodule - but more likely lymph node or pseudonodule on the left that is small. If there is a question of parathyroid, then calcium blood test may be appropriate.
