thyroid or adrenal gland problem/fatigue or both?

don't know what to do, nobody listens to my sypmtoms
Dx in 01 with hashimotos. tried a variety of meds , nothing works, severe heat intolarance from even 1/4 of the smallest dosage along with severe flushing and accesivly hungry, weight gain, feels angry, jump out of skin, facial hair and some on neck/throat. voice changes, and voice sound angry when not.  ear aches, sore troat.  my latest thyroid tests are T4 free 0.8, t3 free 264. TSH 17.35.  have been in 35 TSH before.  feel best when sick, body temp' goes up from 96-97 to 98 which feels like a fever to me.  have been off meds for 2 yrs or so and taking vitamines, iron, fish oil and niacine (colestrol high).  got my first ultrasound of thyroid gland in 09,finding are: in inferolateral thyroid lobe, a hypoechoic nonecalcified noudle (7x7x8)w/out calcification w/ mild peripheral vascularity. In the inferior right thyroid lobe also a hypoechoic nonecalcified, sharply circumscribed (6x7x9) noudule w/ mild peripheral vascularity.  no fluid or perithyroid lymphadenopathy. =benign adenomas.  
my doc, internal med, said to take meds to supress growth of noudle otherwise it will lead to surgery.  however since unable to tolarate meds what to do? my endocronologist gave up, she said I don't know what to do with you and that maybe I have a sleeping disorder and is aware of my intolrance to meds/hives from it.  was at er due to severe side effects from meds yet thyroid level was still low.
Also have ITP, enlarged spleen,off and on, low platlate count and elvated lever enzym.  
my lips, and hands are dry as well as my eyes and throat, sometimes have a burst of energy and can't stop and my speech will become fast and sometimes speech is slurry, or I can't speak like I don't even want to speak or no energy to speak.  I am trying varoise meds from the health food store and haven't found the right one for me yet, though just started.  
What does my test results mean on the blood work and my thyroid ultrasound? nobody explain nothing to me.    

p.s. ear ach.
        feeling tired out of the eye, energy draining of body out of eyes.
        or
        that my eyes can't keep up with the rest of my body is looking    at.
        dislike bright colors, look at colors-shopping.        
funny fatigue that I can't find things when I am looking for something and its right in front of me. hard to look up.
head want to tilt back sometimes/spasm in back of the head and /or legs and or cramps.  
the only thing holding me is multi vitamine and l-throsine but doesn't quiet fix my fatigue.  but my hands and feet don't feel/get cold in ac since taking l-throsine.  can't tolarare full dosage or any herbal stuff at all.
tried cytomel, feel funny.
BEEN off meds for 2 years and doing somewhat better.  looking for alternative meds?
one internalist said I need progestrone, cream?
anybody out there have funny stuff like me?
