if all T4 and 3 are normal but the tsh is elevated do you still need to be treated fro hypo?

I think you've been on a rollercoaster because your meds were adjusted in such wild swings.  First you were increased from 50 to 75 (a 50% increase, not to mention you hadn't been taking the 50 mcg regularly), then you were taken off it completely, then back on again.  Furthermore, your labs are being done much too close together and too soon after a meds change, so they really don't reflect accurately on your meds dosages.  Even your latest labs are too soon after discontinuing to reflect an unmedicated status.  

You've been off meds for about a week.  I'd try the 25 mcg again, then I'd build up slowly.  Try 25 (now that your levels will have gone down from stopping meds).  If you have no hyper symptoms after a couple of weeks, increase to 37.5.  Wait 4-5 weeks after a meds change to be retested.

Are those free T3 and free T4 or total T3 and total T4?

I have been a roller coaster since the beginning of Sept.  I had been on 50 of Eltroxin Since May 09 TSH 7.1, and my hair kept falling out so he increased my med to Synthroid 75 on Sept 4th.  I know  my TSH on Aug 25 was 5.67.  But will have to admit was terrible at taking my med every day but felt great all summer except the hair issue.
After starting on the 75 I felt terrible short of breath and couldnt sleep. So on Sept 8th he pulled me off of it completely for a week thinking it was the change from Eltroxin to Synthroid. then I started to feel hypo again.  Back on 25 on Sept 14th.  My TSH on Sept 22 1.6

On Oct 1th I was cold,backpain and night sweats set in-I'm 38   then on the 19th shortness of breath started with all other symptoms
On the 20th i said the heck with my thyroid med and decided to start from scratch.
My results from Oct 22 were TSH 3.11 T3 4.2 normal 2.6-5.7 and T4 13.6 normal 9-19

so now as of today I;m sure i'm hypo!

What were your exact free t3 adn free t4 numbers?? Normal range doesn't necessarily mean normal.
What was your TSH as well?
