Acidic pH

I have been experiencing low blood pressure, bad breath (horrible), high chloride levels 112, low potassium 3.5, and low CO2 levels 15. I had an ABG done and it came back with my pH 7.37, HCO3 20, BE at -4.4 (very low). I'd had these leves and the symptoms above for about 2 years now, the Nephrologist I am seeing thinks sodium bicarbonate is the desired treatment and I started it a week and a half ago but I haven't seen a change at all.  I would think that the bad breath would have cleared by now since my pH has been alkaline with the the bicarbonate treatment yet it hasn't.  Will it take longer or could the acidosis be related to something else. I am so confused. I am taking 650mcgs every 4 hours.
