Am I only in world with this? Please help!

I am in terrible situation. I am male age 56, was fit, a forester. I am now in great pain,  hardly able to walk, have twtiches, tingling, burning, muscle spams and fatigue.  His story and symptoms in order best I  can recall from easry this year.
Jan-Feb. had probabe epdidymitis
treated with doxy then cipro
About march developed pain in left bicep hard to use arm or raise it
late May orthi doc xrayed say nothing, tried 7 days predisone, said maybe tendon tear ( cipro?)
June pain went to other arm bicep and forearm some
Then had twtiches start, arms now all over.
Then in jume fingers tips numb, then tingling, then toes did same.
By early July getting pain in leg muscles, fatigue, hard to walk more than few minutes, tingling and pins and needls
Noticed losing muscle in arms too, very scared.
Family doctor did full bloof work, thyroid and lyme as I am forester. all neg but 1 panel of  on lyme positive.
The he did EKG, nor mal, sed rate OK and testostorone which was some low, 314 and need 350 but I am 56.
Went to neuro doct two weeks ago. He ddi office checks, said strength OK, muscle loss may be to in activity as i cannot do anything b ut lay in ned, sit ans maybe walk 5 minutes.
he went on and order MRI of brin and neck but not done yet we we have very high deductable and will have to pay all of it. also ordered blood tesr on b12 and gullain barre and cpk? results not in yet.
As Of now I have terrible sabbing pain in right calf, legs feels numb and tingkle, feel light headed, cannot walk far or climb steps much due to some fatigue but every time i do much with legs or arms if they do not hurt right then they will start later and  hurt for hours or all night. The pain in my calf is like level 7-8 out of ten. feels like knife going it but comes and goes some.
I am taking Lyrcia if case its Fibro and Paxil as I have histiory of anxiety and panic with rapid heart beat.
With the twtiches and and I also get some full muscle spasm I am scared of ALS yet I cannot find that pain is a sympom of that  disease as well as sensory symtoms like tingling, burning and pins and needles in legs an arms. Lord know I have had plenty of pain and all that type peripheral neuropathy symtoms. My so called tendon tear in left arm will not heal and it hurts to use it much at all after months of pain there.  I have searched the net but cannot find anything that can cause all this pain, fatigue and neuro symptoms and my docs are baffled. Fibro, lyme, Cipro damage, neuro disease, brain tumor, none seem to  fit. Any ideas that will not scare me more than I am?  I hope for  Fibromyalia (fibromyalgia)  but can it possible do all this? I dod get other pain in thighs, back of things, toes, bicep, foreams and even aroubd eyes. I pray for relief and some kind of hope as I have 11 year old daughter and cannot play with her now and my condition scares her and my wife who is a nurse. Please help or support.
