Any Ideas?

Ok so here is a background..30 yrs old. (5'7", 258lbs) 2 children born 1998, 2002. Tubal ligation in 2002...4 weeks after giving birth. Clip method.  Last mentrual period 4-18-10.  Symptoms: nausea, fatigue, get dizzy easy, breast tenderness, mood swings, and within the last two days I have noticed ligament pain near my hips and my tummy is getting firmer in my uterus area.    Had labs drawn 7/12/10.  pregnancy test negative. estradiol 50 pg/ml. LH 5.6mlu/ml.  FSH 4.71mlu/ml. Prolactin 10.1 ng/ml. Testosterone 15 ng/dl.  Insulin serum 33 uU/ml.
Doctor said it appears I am insulin resistant and put me on metformin 500mg (twice daily).
I still exhibit signs of pregnancy (my original worry) and my doctor said the the metformin would make my M.P. return..which it has not.  Aside from saying I was insulin resistant he has not given me much information. I return to see him 8-30-10 but my symptoms are driving me crazy because I sincerely FEEL pregnant.    Anyone have any ideas on what else could be going on...or what else I might ask to have checked????

I have had the blood preg test and it was neg. I don't have a gall bladder so its not that. My doctor told me my hormone levels are completely normal for mid cycle except the insulin serum. I do not smoke. Never have. not anemic, or malnurished. Weight has been stable for over a year. and my thyroid tests come back normal also. Never had a d&c either.               So I guess that kinda narrows the list down but still doesnt explain what is going on with me. I hate not knowing. After having two children I can honestly say I really DO feel pregnant. Which just makes me feel like I am crazy.

You have the classic symptoms of pregnancy. Hence first get an ultrasound or a blood pregnancy test for confirmation.
If you are not pregnant then you should look for other causes. Sometimes a craving for a child leads a woman to experience pregnancy like symptoms. Liver (hepatitis), gall bladder and kidney problems too can cause these symptoms. IBS, acidity and GERD too should be ruled out. Hormone imbalance (you have this), prolactinomas and HIV too are the other possibilities.
MRI of brain may be required to rule out pituitary tumors secreting prolactin. Another major contributor is polycystic ovarian syndrome (PCOS). This can be diagnosed by a CT scan or abdominal ultrasound and tests for insulin resistance. Smoking is also often associated with secondary amenorrhea and can be worse in presence of stress. Sometimes tumors of the ovaries and brain may also cause absence of menstruation. Severe anemia and malnutrition, excess weight gain or weight loss, hypothyroidism too can contribute.
Another possibility is Asherman’s Syndrome, where there is dilatation and curettage to control bleeding or infection. If you have a history, then this should be seriously considered.
