Brother dysphagia issues

My brother been dealing with  chest pains, chest spasms, burning in the chest , trouble swallowing for the past 18 months . A year ago they found out he had strictures in his throat and they took a biopsy not sure if it was just his throat or if they included the esphogaus . He has had surgical dilations done like more than 6 times of his esphogaus it seems like it helps him for just a tiny bit but not a full time fix and he's on ppi which don't seem to help with that either.
It makes me think since he's getting worse that the dr are missing something so it makes me wonder did they only biopsy the stricture or just part of the throat or was it too small of a sample that resulted in a negative ? Did they not even do a biopsy of the esphogaus but if they did could that biopsy had been too small sample  too result in a negative ?
Or is it possibly something else and what could it be if its something else that is related this ?
