Chest Pain / Undiagnosed

I am a 46 year old male with chronic, undiagnosed, upper left quadrant chest pain. It stays at a level 3 most of the time, but spikes to a 10 for hours at a time. This has been ongoing for about one year. It is a severe "squeezing" pressure just to the left of the sternum that radiates into the left upper chest and through to the back.
I have had several EKGs, nuclear stress test, chest CT, heads up tilt table test, sleep apnea test, upper endoscopy, colonoscopy, a gall bladder function test, and a host of blood tests. LVEF was 67% and all other tests were unremarkable. ( Also had a heart cath - no blockages  ) Gall bladder found to have a very low EF and was removed.
All of this is great, except I still suffer from extreme chest pain, even as I type this. I also had a T-Spine MRI that showed some dehydration and a minor disc bulge at T-9.
I am 6' tall and 200 pounds, in fairly decent shape. I am not stressed out, nor do I suffer from panic attacks. I am a non-smoker, non-drug user, and a light beer drinker. The pain is very real.
Any ideas or thoughts?
