Daily nausea, fatigue, stomach pain. Occasional diarrhea. Ongoing for ~5 years

So I've had a long history of anxiety. Usually manifests itself as nausea and vomiting, followed by the extreme desire to sleep. I think back and I can remember episodes since I was 3 years old, but I had no idea what it was back then. It didn't become a consistent occurence till I was about 18, then it happened every day for about 2 years. Faded slightly for a year or so but that was mainly because I removed everything in my live that could cause stress. Now it's coming back and my life has been on hold for what seems like forever.

The main trigger is fear of being sick, so it feeds itself. If I'm in a situation where throwing up would be a bad thing (out at a social function, eating a meal, etc) that's guaranteed to trigger it. I've been on domperidone, escitalopram, propanolol and had 2 months worth of Cognitive Behavioural Therapy. No effect. The propanolol made me happy for no reason but I could still feel my stomach pain.

The reason this is in undiagnosed is because I'm got the wishful thinking that maybe I was misdiagnosed, or the stress is excacerbating an underlying illness. I've had a blood test but nothing came back positive. It's been years and no real change in my condition. Is there anything aside from panic attacks that could explain all this?

There are two main possibilities. One is that it is due to a medical cause. The other is that it is psychological. There is a posibility that you have an inflammatory bowel condition such as ulcerative colitis, Crohn’s disease, Celiac disease (gluten intolerance), lactose intolerance or IBS or irritable bowel syndrome. Severe acidity with gastric reflux can also be a cause. Liver function tests should also be done. Please consult a gastroenterologist or a GI doctor regarding this.
Improper sleep pattern too can be a cause. Extreme anxiety and stress too is a possibility and hence get a psychological evaluation done.

Even when they think you are an anxious person, they really are doing you an injustice if they do an unthorough search of potential medical causes to your symptoms.  It sounds like there is an anxious component at least from what you are saying.  You only mentioned a blood test.  What did they test?  

Did they test your endocrine hormones, such as your TSH, free T 3 and T 4?  
Did they take a look at your GI system, including an endoscopy to look at your hurting stomach?  
Did they scan your gall bladder and liver to see they are functioning properly?
Did they test your plasma free metanephrines (fasting blood test) or at least a 24 hour urine metanephrine test?  
Did they check you for celiac disease?
Did they ever do a scan of your head to check for a tumor there?
