Dizzyness, lightheaded

Hi, I'm a 21 year old male. About 2 years ago I started getting these dizzyspells, It would feel as if I was dreaming or something and not actually where I was... It would affect my concentration and my eyes would twitch. I went to the doctors so many times and they suggested I had something different every time, Ear infections, low iron levels, stress, bad eye sight, vertigo, cronic fatigue syndrome etc etc. I know i'm not stressed as I have nothing to be stressed about. If anything i'm more stressed about this constant feeling!! I tried everything they suggested to help and nothing worked. Eventually they basicly sat me down and told me NOTHING IS WRONG WITH YOU, they even sent me to a brain specalist just to prove to me there was nothing wrong. He ran a bunch of tests and even took a scan of my head. He coulnd't find anything wrong either. He then sent me a letter asking me if I would like him to go over my results with me. I thought 'why bother...' and never went.

After about a year they got less and less, from time to time i'd still have an odd spell but on the whole it was ok.
Recently they have started again, a really bad dizzy, spaced out feeling that lasts the whole day. It's most noticable when i'm at work and trying to concentrate (I work in a toy store). Please can someone help me.... it gets so bad sometimes I just want to cry with frustration, everyone thinks i'm just being a hypocondriact and doesn't really care. Does anyone else feel like this??

I'm going to give this a couple of days before I go to the doctors again, as it's only started again recently. If I still feel bad then i'm going to go and talk to them about all the things you have posted for me.

There are many causes of dizziness, lightheadedness, passing out, blackouts etc. It is difficult to comment without examination. I know you have been through a number of tests. I am just enumerating the possible causes. You can go over this and discuss this with your doctor and get yourself examined for some thing that has not yet been tested for.
Dizziness could be due to postural hypotension (blood pressure falling with change of position), internal ear problems and cervical spondylitis or compression of cervical spinal nerves. All these cause dizziness when you get up from lying down position or when you change position that results in a change in the level of your head. Cervical spondylitis and ear problems also cause dizziness on changing the position of the neck. What is missing is probably a MRI of cervical spine in your case.
Other causes are anemia, claustrophobia, GERD, hypoglycemia or low sugar and some endocrine problems. Endocrine problems, such as an underactive thyroid (called hypothyroidism), overactive thyroid (called hyperthyroidism), adrenal insufficiency (called Addison's disease), low blood sugar levels, and in some cases, diabetes, may lead to hypotension and hence lightheadedness and an off balance feeling.
Certain types of epilepsies and seizures too can cause similar symptoms. If you have an accompanying headache then you must also look at benign intracranial hypertension. Heart problems too should be ruled out in your case. If all other tests are normal, maybe an ECHO should be done to rule out mitral valve prolapse.
There are various causes of hypoglycemia, postural hypotension etc which will have to be looked into in detail.
