Longstanding right hip pain (groin area) and lower right sided back pain.

I have had the above pain for the past 13 years, it seemed to come on after an accident where I fell from a trampoline but I cannot be specific that this was the cause. The pain in the hip/groin seems to be there all of the time but every six months to a year it "flares" up causing acute pain usually for two days or so then seems to completely go as quickly as it comes on! Weird I know!! Although I do have a pain in the hip usually, and limited movement in the right side the left side seems to be fine. The associated back pain is a newer problem it is on the right "dimple" on the bottom of the back, causing the whole right buttock to ache. Sometimes I can hear my back clicking on certain movements. Any ideas anyone?
