Outside right arm pain just below shoulder

I have severe pain just below shoulder of right outside arm where it feels like nothing there but bone. Twist my arm and the pain on the scale of 1 to 10 is a 20 with it being the worst mainly at night with my whole arm hard as a rock and tingling and numbness in fingers often. Doctors all say that it's nothing but something I'm using to get pain meds. WRONG I can't handle it much longer and say often that I wish it could be cut off its so bad. It resembles this time when I was a teenager that we where seeing who could throw a rock across the Snakeriver and I threw so hard that it felt like I ripped the muscle right off the bone and it hurt in the same place and way for many many years. Any advice or thoughts of what it could be. Thanks to all.

X-ray of humerus might be a good idea at this stage.

Does applying heat help relieve some of the pain?

Does harsh massage (eg. oiling the skin and rolling something firmly along the surface) help?

Does (while very uncomfortable) heavy use, lead to it later feeling not so painful?
