Hi
Welcome to the MedHelp forum!
First of all rush to an emergency to rule out a heart attack. The other thing is to rule out cervical nerve root compression and to rule out sleep disorders. If the ECG and other findings are normal then these two should be looked into.
Another possibility is TMJ or temporomandibular joint inflammation.
Hope this helps. Do let me know if there is any thing else and keep me posted. Take care!

Go to the emergency room, please. The symptoms you list could be caused by a heart attack.  You should get your heart checked!
