Please help, me and the doc are stumped.

The Endo and I are stumped, please help:

Stress Event 2005-2010 (Divorce with Custody Issues) Female, 40 y/o
2006-2009 symptoms, which are now gone:
Sleepy all the time
Falling asleep while driving
No new hair or nail growth
Confusion
2006-now symptoms:
Intolerance to temperature
Cold – Run A/C at 78 degrees, wear socks to bed, 4 blankets
Heat Stroke 3 times
Gained 30 pounds from 2005 – 2008 (155 to 185, size 10 to 14)
Mommy Make-over and strict diet = now 170-175, size 12
Diet restrictions since 2008, minimal carbs and dairy
Can maintain weight, w/o restrictions, gain weight
Moderate sustained exercise, but maximum effort exhausts me
Rapid Pulse
Blood Pressure 110/80, 110/65 range
Sometimes panic attack symptoms wo cause
Psoriasis, Eczema, dry skin, dry hair, brittle flaking nails
     Take Biotin and Zinc supplements
Drink several glasses of water/day… still thirsty
Urine usually yellow
Constipated, take Senna tea to regulate
Acid Reflux often
Frequent Sinus headaches, take 20 mg Loratidine daily
Usually get 7 hours solid sleep nightly
Charlie horses maybe 2x/month
Work 3 jobs
Don’t drink smoke or date
7/23/2011 – cut yard in morning, sweated a lot, was exhausted, dizzy, ate salty chips, Gatorade, and bananas to try to stabilize, weak muscles.
Tired, sleepy, sleeping too much since 7/10/11

Tests so fair:
Sleep apnea: false
Thyroid ultrasound: Fine
Blood tests for thyroid: within Qwest range
Blood Tests:
2008 found:
Low Magnesium
Low Iron
High end of range blood sugar
Very low Vitamin D (mid west)
Low DHEA
Low Testosterone
2010 found:
All parameters within range.

Being treated symptomatically,
Taking daily:
Concerta 36 mg
Loratindine 20 mg
Metformin, 1850 mg
Armor 90 mg
Tylenol, Advil, Tums
Less than regularly:
Biotin, Zinc
Cinnamon
Vitamin D
Topicals for skin flare-ups.

Thanks for the detail, there was one item that jumped out at me while looking at your med list: Metformin 1850mg.  (You did not mention that your a type 2 diabetic)  Metformin is a great drug but has many potential interactions with other medication, herbal supplements, and over the counter products.  I want you to pay close attention to the following safety comments that they put in their own literature (Pub Med Health) and count how many of the side effects listed match your complaints:  

"You may experience hypoglycemia (low blood sugar) while you are taking this medication. Your doctor will tell you what you should do if you develop hypoglycemia. He or she may tell you to check your blood sugar, eat or drink a food or beverage that contains sugar, such as hard candy or fruit juice, or get medical care. Follow these directions carefully if you have any of the following symptoms of hypoglycemia:

    feeling cold    

    shakiness

    dizziness or lightheadedness

    sweating

    nervousness or irritability

    sudden changes in behavior or mood

    headache

    numbness or tingling around the mouth

    weakness

    pale skin

    hunger

    clumsy or jerky movements

If hypoglycemia is not treated, severe symptoms may develop. Be sure that your family, friends, and other people who spend time with you know that if you have any of the following symptoms, they should get medical treatment for you immediately:

    confusion

    seizures

    loss of consciousness

Call your doctor immediately if you have any of the following symptoms of hyperglycemia (high blood sugar):

    extreme thirst

    frequent urination

    extreme hunger

    weakness

    blurred vision

If high blood sugar is not treated, a serious, life-threatening condition called diabetic ketoacidosis could develop. Call your doctor immediately if you have any of these symptoms:

    dry mouth

    nausea and vomiting

    shortness of breath

    breath that smells fruity

    decreased consciousness

Metformin may cause side effects. Tell your doctor if any of these symptoms are severe, do not go away, go away and come back, or do not begin for some time after you begin taking metformin:

    diarrhea

    bloating

    stomach pain

    gas

    constipation

    unpleasant metallic taste in mouth

    heartburn

    headache

    sneezing

    cough

    runny nose

    flushing of the skin

    nail changes

    muscle pain  "

  Numerous items on this list fit your complaints.  You might want to start here and work your way forward with your specialist. I tend to see these types of drug/drug interaction problems on a regular basis. Many times drugs are added to fix a problem instead of checking to see if there is a conflict between existing medications, or sensitivity to any given medication.

Im sorry I am going to be honest here and say what is on my mind... Your mental well being can actually make you phsyically sick. Go out have some fun let loose stop worrying! Stress is the cause of ALL your problems I would put money on it and I dont gamble!

Have you had your hormones checked? Your hormones control sleep, hair growth, etc... You may have too much epinepherine or to much estergen, or not enough testosterone.

Magnesium and Iron can effect your muscles. High blood sugar can cause excessive thirst, frequent urination, etc...

I would have them look at the Vitamin B complex also to see if you may have a B12 deficiency.

Take breaks to let your body rest. Good lcuk
