Vitamin deficiencies, unexplained fever, and feeling awful

Does anyone know what causes severe B12 deficiency and vitamin d deficiency?  I am starting 50,000 IU tomorrow (twice a week) and I will start the B 12 shots (twice a week) for probably the rest of my life.  I have been really sick and have some other symptoms that I have had a lot of trouble addressing.  i have an unexplained fever and Blood Pressure that goes really high(180/120) and then drops really low (90/50).  It only does this for about 15-20 minutes and it is random, but usually every day, multiple times a day.  I have NEVER had high blood pressure before, but it seems to be unexplained.  I also have a VERY rapid pulse of 150.  It is always at least 100 even when I am laying down or doing nothing.  I am having blood work to test for hormone probs, but i just wondered what different things can cause these 2 vitamin deficiencies as I don't believe they both just happened randomly.  Can anyone help?  I am desperate...

ANA was neg, EKG was normal, chest x-ray was fine, mri of brain with and without contrast was fine, head ct was fine, vit d defiency less than 7, b12 was almost 200 (so very deficient), normal BUN and creatnine, normal c- reactive protein, and normal sed. rate.

I have decreased urination, severe fluid retention, BP going up and down for 15 minutes at a time, very rapid pulse, headache (severe), dilated pupils, anxiety - did not have before, joint pains, unexplained fever, numbness in right pinky finger and in toes on right foot, a rash on my arms, and severe sweating.  Any help would really help me.

My B-12 deficiency can be attributed to alcoholism.  I have 6 mo sober and still need shots.  There could also be a hereditary factor, my grandma has pernicious anemia, meaning she cannot absorb B-12 normally and needs shots.  Also, any stomach trauma or surgery will affect the way your body absorbs it.

Causes of vitamin B12 deficiency include not enough vitamin B12 in diet (rare except with a strict vegetarian diet) ,poor absorption (malabsorption) diseases (for example, celiac disease and sprue) ,lack of intrinsic factor,hypermetabolic state for example, with hyperthyroidism and pregnancy.

Causes of vit D deficiency are inadequate intake coupled with inadequate sunlight exposure, disorders that limit its absorption, conditions that impair conversion of vitamin D into active metabolites, such as liver or kidney disorders or parathyroid disorders or rarely, by a number of hereditary disorders.
