What can this be?

My fiance's been having a continuous terrible headache fot the last few days, he was taken to the E.R. the other night as his blood pressure went up to 170; his back aches as well and as of two days he's been having very high fever too;  they sent him back home but yesterday his fever and blood preassure went up again; they ruled out malaria; what can it be then? Sounds like there's a severe infection somewhere, but although they ran many blood tests on him, they couldn't come up with a conclusion yet; why is his blood pressure so high anyway? He lives in Italy by the way and he's Nigerian, if this can help. Thanks.

Has he had any cuts recently that may be infected?  I know that doctors don't look over the whole body... I would think that his high fever could make his blood pressure go up.

May be menengitis. Did they do a spinal tap?
