Years of pain, need help diagnosing and/or finding doctors to help the problem

I never actually thought I would be writing this, but after experiencing this through medications/treatments/diagnoses, I think I need help from people other than the ones that I know. I am a 15-year-old girl, and for the past three or so years, I've been experiencing heavy and irregular menstrual cycles, with cramps so bad throughout my entire cycle that I've missed over fifty days of school over the course of three grades, and they've made surviving through the average day unbearable.

I've had my menstrual since March of 2012. At first, it was regular in all terms of the word that can be described for just getting it, but over the course of a year it turned into my worst week of the month. My cycles have lasted for over 30 days without the use of hormone supplements, and the menstrual itself has usually occurred for seven to nine days. At the beginning, I was told it was normal, but then new symptoms started popping up, such as migraines that also accounted for 12+ days of missed school in the 7th grade, nausea, heartburn so bad that even prescription-strength relievers wouldn't help, and a resurge of insomnia (which I've had since I was very little) and hypersomnia at the same time.

As the pain became worse, I visited the ER around three times before I was taken to a rheumatologist due to blood work that showed an autoimmune recessive trait. Said rheumatologist diagnosed me with Amplified Pain Syndrome (APS or PAS), which is where the nerves lose function of determining pain and, as such, pain can be felt by even the slightest of touches. She recommended I take anti-depressants (which is an absolute no-no for me, according to my therapist), and physical therapy, the latter of which I took six weeks of and it made the pain even worse.

I've also had problems with bladder infections almost every single time I've had a menstrual, and after an accident earlier this year, my pediatrician took some more blood work and sent me off to a gynecologist. The gynecologist has thought that the pain has been a result of possibly endometriosis, and prescribed me some Naproxen and mild hormone supplements/birth control (Spirtex). However, the Naproxen hasn't helped, and the birth control has only relieved the pain mildly over the past three months I've been taking it.

Four weeks ago, I also got a new symptom, and it's been this rash on my palms and on my arms. It appears to be hives on my palms, and I've itched so much that the skin has been peeled off. I've tried Zirtec, Benadryl, and extra-strength, prescription anti-itching cream, but nothing has been helping. I don't have any allergies whatsoever, and haven't been exposed to parasites, or poison ivy, oak, or any other poisonous plants.

I'm asking this as somebody who is done dealing with the pain, and as somebody who is also done with taking the "you're a hypochondriac" answer from members of my family and close friends. I've been told by my doctors that I shouldn't be experiencing these symptoms at 12-15 years old, but they have no idea how to treat it and no idea how to diagnose it. I've used all of the mentioned medicines/treatments above along with a heating pad on my lower back for my cramps, and they've all haven't done much. PLEASE, if you know of anybody else who's experienced this and has gotten a correct/working diagnosis, or if you know a doctor (hopefully in the Ohio area, but anywhere in the entire US or, hell, even the world is fine) that could be willing to take on my case, let me know. I'm tired of this pain, and I'm tired of being a zebra in a herd of horses.
