Dear Dr, I have no where else to turn. I live in canada and I'm 29 years old. Family history of type 1 & 2 diabetes and hyperthyroid. On aug 2007 I presented with symptoms that were rather scary. I had a rash on the right side of my penis. Also with fever and I was feeling very tired. I went to a walk in (as i don't have a family Dr.) And the GP said that the rash looked bacterial, she gave me fucidin and sent me on my way. She said if after the rash disappears I get pain like pins and needles then its shingles. Thats exactly what happened for five months and i still get that pain from time to time on the right side of my penis shaft. Not to mention that the right lymph node had swelled and still is with moderate pain. I had an affair that ended in June of that year and I was convinced that I had hiv or hep. I was screened for everything from hiv to the clap.Everything was fine, and yes I did wait 6 months before getting tested again and everything was still fine. Actually all tests were fine. I've seen 2 rheumatologists and a infectious disease pract. and they say there are no signs of disease.....? The only tests to come out positive were ESR was 30. Lymphocytes was 3.9 and wbc 11.0 and a abdominal ultrasound showed a 2 cm growth on the liver, 4 months later the growth was gone, and build up of plaque in the sinuses membrane. (did i mention I smoke 1 pack a day)Also a eye test showed loss of colour vision. Oral thrush on tongue. And absolutely no reflex on right foot.
From august 2007 till now I'm still sick. I have migrating muscle twitches, headaches that shoot to my right eye, loss of colour vision, chronic sinus pain. sore lymph nodes on neck, tightness under the chin. ear pain, oral thrush on tongue, Joint and muscle aches especially on right shoulder, elbow and hands. Feeling of pins and needles on hands and feet. Periods switching from constipation to diarrhea, liver pain. Serious pain where lymph node is swollen on right side of pelvis. Burning when urinating. Increased urination. Excessive hunger, weight gain. Did I mention diabetes and thyroid tests were within normal limits. Please help me or point me in the right direction. Good to know you don’t have any oral thrush.

Honestly, speaking, I don’t think you’re suffering from an immune deficiency disorder, although you do have problems but probably nothing to do with impaired immunity. Although, I can’t think of a single diagnosis which can correlate all your problems but I feel that your problems are not severe enough to trouble you lots. You just need some willpower to overcome those. I don’t think you have a significant urinary problem or a significant back problem to require any urgent attention. Please don’t worry about the penis thing also.

Some regular exercises like a walk and a more positive out look towards life will probably help you more than too often doctors’ clinics.

Good news, I went to the ear, nose and throat specialist yesterday and he ruled out oral thrush. He said I have smokers tongue and nasal and sinus polyps. Also my nose has a curve in it and I might have to be operated on. He prescribed me Nasonex. That explains why my sinuses have been bothering me for so long. Does this rule out an immune defiency now? Please let me know.

Hi, thank you for your reply. It's nice to know someone is listening. Your reply does help, an immune disorder can't be ruled out. I've had sinus problems since I was a kid. Chronic nose bleeds and facial pain and recurring lung infections starting in my early 20's. As much as three times per year.  In 2003 I suffered a workplace injury which resulted in a minor concussion. During my CT scan the woman asked me if I was feeling any sinus pain. I told her no.( I wasn't). She said how could you not feel that your sinusus are so inflammed. She also added that there was nothing else wrong but just inflammed.

Before I shorten my list of symptoms I wanted to add that I also sufferd with chronic low back pain since my early 20's. I had an A-C right shoulder separation which I felt never healed right. I don't know if this is relevant but I thought I might add them just in case. Also about 6 months leading up to when I got sick I was experiencing severe night sweats with a dull pain and numbness in my right shoulder. My wife would complain about the bad odor of my sweat. My right ear got so plugged with wax I couldn't hear. The GP had to flush it out and he said I had an ear infection. I always clean my ears.

My worst symptoms are pelvic pain especially on my right side where the lymph area is swollen and the pain tends to radiate down to my foot and around my hip to my lower back. I also feel a burn on the right side of my penis where that vein is.( I don't know the correct term for this)Burning when I urinate which only happens when everything else hurts and I have had urinalysis done during this burning and the tests were negative. Also when Im not constipated I have Diarrhea. Liver pain, dull joint pain, weakness in my arms and legs, sinus pain, and fatigue.

Hi,

I am really concerned for you. I know you are in trouble. Most of the blood tests you’ve mentioned are pretty okay except the mild rise of the ESR. I’ll request you to please mention the symptoms which trouble you the most so that I can think of some correlation and we can reach to a conclusive diagnosis as the symptoms you have mentioned are quite varied and such varied symptoms occur in patients who are HIV+ve which you’re not, in diabetics (excessive urination, excessive hunger etc) but your blood sugar is normal. A possibility of an underlying immune deficiency disorder can’t be ruled out(rash, oral thrush, sinus problem,enlarged,sore lymph nodes etc). You may need to see an immunologist to find out the cause.

Meanwhile, it’ll be great if you can abridge your symptoms list and let me think more on the same.
