microscopic blood in urine

I have had microscopic blood in urine for 2 years.  I have underwent the cysteroscopy and also a complete scan (US retroperitoneal Complete) and both came out okay.  I am going back to the nephrologist next Tuesday and will see what he says. (I have seen 2 nephrologist in the past 2 years and both said that under the microscope they don't see any red blood cells but that was about 1 1/2 years ago)  Needless to say, I am concerned since whenever I mention to anyone I have blood in the urine they immediately say I probably have cancer or something as serious as that.  I have had blood work and urinalysis done over 6 times in the past 2 years and still no diagnosis.  Any help or advice would be greatly appreciated.  Ask me any questions, I am an open book.

Hi mdfarrell - wishing you a happy and healthy 2014.

I am glad to hear from you and that everything is under control.
I occasionally check my urine with the urine sticks when I have pains in my kidney area, occasionally blood is detected and sometimes leucocytes, but then clear in the morning.  Over the last few days things have been fine.

My doctor told me to monitor my blood pressure over two weeks and let him have the results because my blood pressure was far too high when I saw him in December.  My blood pressure has been fluctuating, but nothing too serious - that is until I got a phone call which was not very pleasant and my blood pressure shot up to 203/80.  I took a diazepam to calm me down (I didn't feel worked up though - buy hubby knew better!)

I shall have to take a reading of my blood pressure over the next few days and see how things are (haven't taken it since the high level) and then submit the results to my GP.

Well I finished my work up and had my appointment today.  The Dr. said everything looked good.... and the blood pressure medicine must be working well for my kidneys.  He did increase my dosage because my bottom number is still running a little too high.  The creatine/protein levels came down considerably after being on the meds for only one month. So..... there is not kidney biopsy in my near future, if at all.  

All I am having to do is keep an eye on my blood pressure..... too high, call the dr, too low decrease meds then call the dr.

How have you been since a month ago?  Everything leveling out?  Bring me up to date please.

My blood pressure has come down and isn't so high since the doctor took it.  I will monitor it like he suggested and forward him the results.  May be I should do the same with the traces of blood in my urine!

I keep detecting traces of blood in urine from time to time.   There was some last night, clear this morning, but no leucocytes or protein.  The 24 hour urinalysis seems like a good idea.

You sound like my husband about the salt and onion.... he loves foods that are good for you but he has to have that salt on them.  Thanks for your support!  Stage 1 Kidney Disease is when you have proteinuria and hematuria at low amount with no other major symptoms.... or at least that's what I think I understood.  No I did not want to hear Stage 1 of anything but I knew there had to be a reason for the rollercoaster ride I have been on for the past 2 years. I am curious (and a little scared) of what the 24 hour urinalysis will show.  I am going to hope for the best and work on eating healthy and getting back to my exercise routine.  
I hope your blood pressure stabilizes and you get everything under control!  Best wishes to you, too... Let's keep in touch.  This has really helped me and I hope I can return the favor one day!

Well at last you have some answers, albeit having Stage 1 of kidney disease is not something you would have wanted to hear.

Hope the Ace Inhibitor works for you.

My kidney pain settled down after the antibiotic.  And as always my urine results are always clear when I have to take in a sample.  
I too am at the moment monitory my blood pressure for the next two weeks because it was far too high when I saw the doctor last week.  It has come down to a more acceptable level over the last few days, but still on the raised side.

I also should avoid eating salt and purine foods, but I just love salt - onion doesn't seem to taste quite the same without salt :)

I don't know much about stage 1 kidney disease, so will learn about that.

Well, I saw the nephrologist yesterday and got some answers... finally!  Kidney Disease Stage 1- Stable; Hypertension- uncontrolled...... He changed my blood pressure medicine and I will do a 24 hour urinalysis in 2 weeks to see if the "ace inhibitor" helps with the blood and protein in the urine.  Of course I am now on low sodium diet (not a problem for me since I have watched sodium most of my life)  I don't need to see a urologist again since the scans and cystoscopy all came back fine.  If the change of medicine doesn't work then he will increase it and see what happens..... if all fails then I would have to have a kidney biopsy to see what is going on inside there since that is the only place that hasn't been explored.

Thank you.  Here in the UK we do not have a Thanksgiving Celebration.

Just a tip, don't drink more water than what your urologist has advised.  Drinking too much can be just as dangerous as not drinking enough.  2-3 litres is usually recommended.  But be guided by your urologist or doctor.

I will continue drinking my fluids (I drink lots of water every day- not a soda drinker at all)  I hope your problem settles down for you real soon.  I will keep you informed and I hope you have a very Happy Thanksgiving!

It is a bit worrying, I agree, but nothing much we can do about it apart from drink more fluids.

My problem was quite bad earlier on in the year with on average a monthly prescription of antibiotics for blood, leucocytes and protein in my urine.
By the time I received an appointment to have an ultrasound scan things had settled down.  I knew they had because I felt no niggles in my side.

I already knew that I had a cyst and there was no change in that, but it does play up from time to time.

The problem has started up again recently, so I have upped my intake of fluids with honey and lemon and water, have to keep off peanuts and celery (these foods contain purines that can lead to formation of stones. Persistent urinary infections can also lead to formation of stones.

Yes blood was detected (on a dipstick in the doctor's office) in my urine at my last visit a few weeks ago.  I will feel better once I see the nephrologist and they examine my urine under the microscope.  I would have never known any of this had I not had the condition with my eyes 2 years ago.  That is when all of this started.... I would not know now that I am RA positive, blood in urine, etc...... I have got to find a way to stop worrying about it all.  but it is so easy to say and difficult to do....... Thanks again for listening and responding...  not being a doctor is great as I have seen my share of all different kinds of doctors over the last few years.

That's great that you have no symptoms.
Unfortunately RA is an autoimmune disorder as you already know and your blood test has shown that there are high lives of inflammation.

With any type of arthritis condition, there are good and bad days with joint pains and stiffness.  Having some form of exercise and eating a healthy diet is beneficial.

I am a little confused with regard to your blood in the urine.  So has blood been detected in your urine when you had your last urine test?

Or is it something that is detected from time to time?

I am not a doctor and probably would not know what all your test results mean.  I am aware of some urine problems like I mentioned in my early message due to my own experience and using a reagent test strip.  A laboratory test on the urine is more accurate and gives actual levels.

The thing about the RA is that although I test positive I have no symptoms.... yet.  So I am not treated, in fact, I take no medications for any of this. The main thing my doctor is concerned about is the blood in my urine. So I will see the nephrologist Tuesday and go from there.  You have been a great help and if there was a way I would send you a copy of all my test results and see what you think.  There are three things that my rheumatologist is concerned with: blood in urine, RA positive and P- ANCA is still positive (1:32 is my value and has remained unchanged for 2 years)  Since I have no symptoms the main concern is the blood in urine.....

Have you asked your doctor or the consultant to explain the tests results to you?  If not, then I would ask.

A sugar level of 89 gmg/dl (American) converted to sugar level in mmol/l (British) is 4.94.  This is good and means that you do not have diabetes.

The BUN test measures your kidney and liver function.  BUN levels can increase with the amount of protein in the diet whilst low protein diets can cause low BUN levels.

You should speak with your doctor with regard to the results of your test, but as you have been informed that there is no abnormality, then things are fine.

As you have a positive blood result for rheumatoid arthritis, you should be getting medical help for this.  RA is an autoimmune disorder and affects soft tissues as well as bones.  You may find that certain foods, cold weather, hot weather, stress and anxiety may make your condition worse.  You will also experience good days and bad days with RA.  Ask your doctor if there is a support group that you could be referred to learn how to cope with your RA.

Relaxation and breathing techniques may help you through the rough times.

Yes worrying makes me feel bad and the only comfort I have is that I have actively tried to find out what is going on with me.  If I hadn't had the eye disorder I would not have known anything at all was wrong with me. Thanks for listening

As you had a hysterectomy, that will definitely rule out any menstrual cycle bleeds.

I to do not know what RBC and WBC mean, so I cannot comment on that.

When your urine test showed up protein that indicates that was is an infection.  

I buy the Reagent strips for urinalysis the 10 parameter ones they are fairly cheap to buy through Amazon.  These are what the doctor uses when you make an appointment to check your urine specimen.  A laboratory test is far more precise and accurate.  

These are still good to give you an indication of what is going on.  I have a history of kidney stones and urinary infections so having the parameter test strips allows me to check my urine should I get any symptoms and get medical attention quickly.

Leucocytes that are detected means an infection.
Protein indicates an infection
Nitrite indicates a bladder infection like Cystitis
PH level will show whether your urine is normal or acidic
Blood showing on the test strip with protein and/or Leucocytes indicates a urinary tract infection.

Specific gravity and pH levels can also show if you are not drinking enough fluids.

Glucose levels detected show that there is too much sugar in your body that would indicate diabetes.  The urine test is not accurate for this because it only picks up glucose levels that are fairly high. And does not show the glucose level that is in the blood.  

Your back pains may be due to arthritic changes and could also be from a urinary problem.

If you have twinges in your sides below the rib cage, at the front or the back that could indicate a kidney problem.  I am having a dull ache in the side at the back under my rib cage as I write.  

As there has been no detection of blood under the microscope for some time now and your tests have come back fine, then do not worry.  
If you do get physical symptoms and traces of blood are found in the urine through lab tests and there is also leucocytes and/or protein present, that will mean that you have a urinary tract infection.

When you use the toilet to empty your bowels, wipe from front to back.  Bacterial can easily enter through the urethra.  If you notice a stinking sensation or irritation when you urinate and going to empty your bladder more than usual, that would indicate a bladder infection.  Blood is not always seen in the urine by the naked eye and if you did experience these symptoms, a visit to the doctor with a morning mid flow urine sample would be advised as well as drinking more water.

Make sure that you drink water and other fluids so as not to get dehydrated.  It may also be a good idea for you to have a routine annual blood test to check your kidney and liver function.

As you have had tests and nothing has been found, then things have settled down and there is no need to worry unnecessarily.  Worrying will make you feel unwell.  If you do notice the symptoms that I have mentioned above, then do make an appointment with your doctor.

Thanks for your input jemma!  I have had Occult Blood results that have fluctuated from moderate to large to moderate to 2+.  This is on the urinalysis auto only. I haven't had any symptoms that I know of... I have lower back pain here and there but nothing major.  I have never seen blood in my urine so when they detected it I was baffled. All of this started with my eyes....2 years ago I had what appeared to be pink eye, in fact that's what the opthamologist diagnosed and treated but it didn't get better it got worse.  I went back and saw a cornea specialist who diagnosed me with a corneal melt.  She ordered lots of blood work and it showed I was RA positive so she referred me to a rheumatologist.  When I went to the first rheumatoid dr he did more blood work and a urinalysis which showed protein and blood in my urine. He also said my blood work showed positive for vasculitis and he referred me to a nephrologist to have a kidney biopsy.  The kidney dr said a biopsy wasn't warranted at the time because although the dip stick showed blood under the microscope he didn't see any. I then found a rheumatologist (2nd opinion for the vasculitis) in New Orleans and did the entire workup again.  Another nephrologist confirmed what the first one said which was no blood is seen under the microscope.  Then they sent me to a urologist and I had the cysterscope of the bladder and an ultrasound/echo.... of the kidneys and since everything appeared fine.  This has been a roller coaster ride with no definitive answers therefore no treatment.  As far as the lab testing my urine, I don't know.... if you could tell me exactly what that would be called I can see if I have that one in all these test results.  The only thing out of the "standard range" on my last urinalysis was Occult Blood UA 2+; bacteria UA "rare";   My RBC was 3 and my WBC was 1 which were both in the normal range... I don't understand it all and I guess right now I am just afraid after all I have been through they have missed something.  I find myself waiting and watching for symptoms (which I don't really know what I am looking for) and sometimes my mind plays tricks on me.... this has taken over a huge part of my life.... the waiting and worrying is just taking a toll on me.

When you say that you have had microscopic blood in the urine over the last 2 years, has this been constant, or intermittent?  

When was the last time that you had your urine checked by the laboratory and what did this show?

When women still have menstrual cycles, a little bit of microscopic blood can get into the urine sample.

Sometimes microscopic blood can be detected in the urine if there is a kidney stone anywhere along the urinary tract.  Kidney stones do not cause pain or problems until they start to move or press on tissue.  Kidney stones come in different sizes and can sometimes be very small like a grain of sand, but can still do damage if they scratch the linings.  Some stones (like grains of sand or sediment) can pass through the urinary tract without you knowing that you have had a stone.

Do you get any physical symptoms at the time that microscopic blood is detected?

When I detect traces of blood on my reagent strips for urinalysis, with no leucocytes or protein, I do experience discomfort in my left kidney area and observe my urine for any signs of kidney stones.

Make sure that you drink enough fluids and that you do not become dehydrated.   Drinking fresh lemon juice with water will also help.
