I am a 53. In Sept 2006 I had vaginal hysterectomy due to uterine prolapse, TVT, and rectocele repairs.  Recently I was in for a yearly exam and discovered that I have vaginal vault propase (vaginal apex was at 4 cm while pushing) cystocele grade 2 and rectocele  grade 2.  The Dr stated that he believes that the sutures from the pervious surgery holding my vagina in place have failed.  The Dr recommended a pessary to try and control the damage and inclusion of dietary fiber (metimucel caps) and stool softeners.  He wanted to wait 6 months before deciding to do surgery, but stated that he would probably use mesh between bladder and vagina, rectum and vagina, and reposition my vagina in the correct place via incision above my public bone.  Although I have the cystocele, I do not have incontinenace with that.  I occasionally have fecal incontinence with the rectocele.I have a substantial amount of pelvic pressure and low back pain after being on my feet all day.  No pain with intercourse but a lot the day after.  I was fitted with a Gellhorn pessary.  I have not had much success with it.  I am comfortabe inserting, removing, and cleaning . It seems to fit well and hold my vagina higher then without it, but I get a lot of pressure, rectal pain, and posterior leg pain when I it is in for longer than 4 hours.  Initial insert feels fine and comfortable, but the longer I have it in the more uncomfortable it becomes.  

My question is with all of these issues, previous history, and the understanding that these things normally get worse with time, why would a Dr want to wait 6 more months prior to deciding to initiate surgery? Is the possiblity of more damage that could not be repaired by waiting this additional time? I am thinking about asking my HMO for a second opinion but want to know I am on solid ground prior to doing so.

Where has the Dr gone?  In clinic I suppose.  I posted this Sept. 8, and there has been no reply nor any other message posted since the 8th?
