Surgical Error

I had surgery last week to repair a prolapsed Urethera, bladder and rectum.  While performing the surgery on the rectocele, my doctor mistakenly cut into my rectum.  She then had to call in a colon specialist to repair the damage.  My question is how common is this mistake?  Was this carelessness on the part of my gynecologist?
