pain during sex/during pelvic exam

I've had this problem as far back as 6 years ago after the birth of my first son.  I've had 3 kids and I'm 26 years old.  The latest birth was my 2nd son in 8/05.

Upon deep penetration or during the physical exam where the doctor puts her fingers inside and pushes down on my bladder, it hurts like mad.  Doctor said the pain is from my bladder, not my uterus.

My gyn did the normal exam, pap, smears, and cultures (vag & bladder) all normal, pelvic ultrasound which only showed a small cyst on each ovary.  She said the pain isnt from that.  No other advice was given.  The nurse told me that I may want to see a urologist even with negative findings.

My periods have been getting shorter in duration, acne which I never had before my last birth, severe pms, and the pain has just gotten worse since him.  My period in January came on 1/20, this month it came on 2/13.

Any advice on where to turn?  Even the positions that didnt cause pain prior are now starting to hurt.  The pain where I can explain it best, is approximately where the 'g spot' would be on the front wall of my vagina behind my bladder.  I urinate constantly but I drink water all day.

Your pain may be related to pelvic floor muscle dysfunction. If your gynecologic workup is negative, please consider evaluation by a qualified pelvic floor physical therapist.

If you  have blood in your urine (not with a UTI), or if you are a longtime smoker, you may need evaluation by a urologist/urogynecologist. However, you probably should be evaluated by a qualified pelvic floor physical therapist to see if your pain is musculoskeletal in nature.

To: additional info I left out
Also since my son, my periods have been INCREDIBLY heavy and I'm also getting cramps that I never had before.  My ultrasound at the time was done on 2/8 and my uterus was 'thick' and I got my period on 2/13.

There was no blood in my urine sample.  Where would I find a pelvic floor therapist?

Thanks! :)

Look at the University of Pittsburg for a Urogynecologist. They should be listed under the division of OB/Gyn. You want to ask for a "fellowship trained" urogynecologist. From there, you should ask who they use for their pelvic floor physical therapy. Then ask your primary MD for a referral to that physical therapist.

If you are having problems with that approach, send me an email (***@****) and I can better direct you to a PT in your area.
